H.O.M.E.'s 2011 Wreath Order Form 


Retail shipping begins November 28.  Orders received earlier are shipped first.

 
All orders must be received by December 2 to make sure you get it by Christmas day.
You can call after that to order an item to pick up at our Craft Store!

We will enclose a gift card with your name and a choice of greetings if you wish.


2011 Retail Prices (Includes shipping and handling)
                           (Maine residents must add 5% sales tax)
    (Lower 48 states only) 

1. Please read the Instructions on this page so you will fill the form in correctly.

2. Click on Print Order Form and print the form using your browser..

2. Fill it out with your name and street address, along with the names and street addresses
        of those to whom you would like to send our products.  (They cannot be delivered to a PO Box)

3. Be sure to include Street address for truck delivery.       Please include phone numbers so any delivery problems can be quickly solved!

4. Mail this form, along with your payment in U.S. funds, to:
